This section provides information on Unofficial Transcripts, Official Transcripts, and Course History reports. A Course History Report is for law students who attended the University of Wisconsin-Madison for other coursework but do not want that coursework included when listing law school grades.
In most cases, when an employer asks for a transcript, an unofficial one will do. An unofficial transcript simply provides a list of the coursework, grades, credit hours, and overall GPA for the employer and does not contain the University's official seal like an official transcript does.
Below is a template for creating an unofficial transcript in Microsoft Word. Be extremely careful when inputting grade data, as errors can be viewed as carelessness and lack of attention to detail, or even as misrepresentation. Current students can obtain grade information by logging into My UW-Madison, and looking under academics.
Students and alumni should know how to order official transcripts from all secondary educational institutions (undergraduate, graduate, or others attended for credit). An employer, especially judges, may ask for official transcripts as part of the application process. Contact the other institutions you have attended for information on how to obtain official transcripts.
An official UW transcript must be ordered from the UW Registrar's Office, located at 333 East Campus Mall #10101, Madison, WI 53715-1384 (608-262-3811). The Registrar's website for ordering transcripts is: http://ordertranscript.wisc.edu/.

Course History Reports
Students who have previously attended the University of Wisconsin-Madison have the option of requesting from the Law School Registrar, a Law School-produced course history report. Such a report will contain information with respect to courses, credits, and grades from a student's current matricultion period (i.e., Law School courses), but will not contain information from a previous matriculation (e.g., when the law student was an undergraduate). Requests for course history reports must be submitted in writing, on the form used by the Registrar for that purpose, at least two weeks in advance of the date on which the student needs the report.
